Special Needs Plans (SNPs) are a type of MA (Medicare Part C) plan that restricts enrollments to those who meet certain eligibility conditions. The plans provide benefits that serve the certain health needs of its members. There are three types of Special Needs Plans available: Chronic-Condition Special Needs Plans (C-SNP), Institutional Special Needs Plans (I-SNP), and Dual-Eligible Special Needs Plans (D-SNP). In this article, we’ll be focusing on C-SNP.
What are C-SNPs?
This is a type of SNP (Special Needs Plan) that is designed for people with certain disabling conditions. They can provide health insurance benefits for those with a certain chronic health condition or for those who may suffer from many chronic health conditions. At a minimum, this plan must provide the same benefits that all Medicare Advantage plans must provide.
More than just standard Medicare Advantage plans, Chronic-Condition Special Needs Plans aim to help you manage your chronic conditions. Most times, the plans modify their benefits and network to better meet their members needs.
C-SNP Eligibility Requirements
If you are interested in signing up for a C-SNP, you must meet certain qualifications. First, a beneficiary must be currently signed up for Original Medicare. Next, they have to live in a community that provides the service they are looking for. Finally, you must have one or more of the following chronic health conditions.
- Autoimmune disorders
- Cardiovascular disorders
- Chronic alcohol and other substance dependence
- Cancer
- Chronic lung disorders
- Chronic heart failure
- Chronic mental health conditions
- Diabetes mellitus
- Dementia
- End-Stage Renal Disease (ESRD)
- End-stage liver disease
- HIV/AIDS
- Neurologic disorders
- Stroke
- Severe hematologic disorders
If you are suffering from one or more of the chronic health conditions listed above and meet all eligibility for Chronic-Condition Special Needs Plans, you may qualify for C-SNP Medicare.
